Case Management & Community Based Rehabilitation (CBR) Services

These two services are usually provided in conjunction with each other but certainly are not reliant on being carried out that way. They provide closely related options to help improve the function of a client’s life. Currently Medicaid is the only insurance (and Medicaid backed policies) that pays for these types of services. However, we have some clients choose to pay for these services out of pocket without insurance involvement.

Case Management Services provide a worker that is dedicated to advocating for the client, linking them to community resources, and coordinating clients to appointments of all kinds. These services are useful for people with severe mental health issues, physical health issues, complex family/social history, and those needing a guiding hand to get back on track.

Eligible and/or interested individuals meet with a Case Manager to determine specific needs through customized service plans including, but not limited to the following areas of life:

  • Psychiatric medication management
  • Physical Health
  • Financial Health & oversight
  • Housing
  • Vocational / Educational stability and advancement
  • Basic living skills
  • Legal issues

On the other hand, Community Based Rehabilitation Services (also known as Psychosocial Rehabilitative Services) involves teaching clients coping skills, life skills, and about their conditions and symptoms that arise from it/them so that they will be able to live independent and productive lives adding their own personal contribution to society.

A comprehensive, skills building model is used to work with individuals out in the community. Our model of services is custom tailored to each client, helping them to eliminate the dysfunction in their lives. We are a curriculum driven service provider subscribing to some of the most effective research-based treatments available.

Services can but are not limited to include:

  • Symptom management
  • Interpersonal skills training
  • Social and communication skills
  • Budgeting skills
  • Community reintegration
  • Organizational skills

Each client will take an active role in the building of the service plan putting them “in charge” of their treatment and accomplishments.
